Request for Medical Records – Instructions & Information.
If you are a Patient:
Obtain your medical records:
If your visit was on or after April 1, 2024, obtain your medical records via patient portal: Patient Portal
If your visit was before April 1, 2024, please contact the clinic you visited, and they will be able to assist you: Exer Locations
Obtain your medical records In Person or by Mail:
To request your medical records in person, please bring the following required documents with you to your nearest Exer Location:
o Government issued photo identification
o If patient representative, include legal proof of authority to act on behalf of patient
To request your medical records by mail, please mail the following required documents:
o Copy of government issued photo identification
o Signed Authorization for Use or Disclosure of Health Information
o If patient representative, include legal proof of authority to act on behalf of patient

Please note: Unsigned and/or incomplete requests will not be processed and will be returned to requestor.
Subpoena, Copy Service, Law Office or Other Third Party
Please submit the request to the Medical Records Department using one of the methods below:
Electronically via Chartswap:
o Upload a copy of your request
o Check the status of your request
o Make electronic payments

Required Information
All Records requests must comply with HIPAA and other applicable privacy regulations.
Merger Notification
As of November 1, 2023, UrgentMed (including Anaheim Urgent Care, Inc., Advanced Urgent Care of Beverly Hills, Inc., Santa Monica Urgent Care, Inc., and Primary Med) ceased operations, and the clinic locations are now operated by Exer Medical Corporation. Records requests for dates of service November 1, 2023 to the present should be directed to
Exer Urgent Care.
Requests requiring dates of service prior to November 1, 2023 should be addressed to the
appropriate party.
Notice of Third Party
Exer Urgent Care partners with Sharecare to provide copies of medical records for a subset of requests. You will be notified if your request is forwarded to Sharecare.
Medical Record Copy Fee
You will receive an invoice for the production of records by mail/email.
