Exer is excited to announce the opening of several new clinics. Due to these clinics previously being operated by Optum, some patient records may need to be accessed in their historical records system.
Our online patient portal gives you 24-hour access to view historical visit information for any Exer location you may have visited.
Using the guidance below, please select the appropriate link to access your patient records. If you do not see a record you’re looking for, please call the clinic at which you were treated.
