Exer Urgent Care offers appointments for several services
Exer Sports & Schools
Thank you for making your appointment.
Please remember:
- Bring the students school issued sports screening form
- A list of any medications the student is currently taking
- Glasses or contacts if the student requires them
- Students under 18 must be accompanied by a parent or guardian
- Parent or guardian must bring valid ID
- If, for some reason, you cannot make your appointment, please cancel 24 hours in advance.

Pre-Op Exams
Before undergoing major surgical procedures, most surgeons require their patients to complete a Pre-Op Exam.
Please remember to bring:
- Valid ID
- Insurance or credit card
- Your doctor’s order form
- A list of all your medications
- Any recent labs and results
Please arrive promptly and we will do our best to see you as close to your appointed time as possible. We look forward to seeing you!
